Job Overview:We are seeking a skilled Band 7 Occupational Therapist to join our team at King's College Hospital. The role focuses on providing effective case management, conducting comprehensive risk assessments, and delivering targeted treatment to support patients' recovery and enhance their quality of life.

Key Responsibilities:

  • Manage a complex caseload, including conducting case management and assessments for patient needs.
  • Perform home visits and risk assessments to ensure safety and continuity of care.
  • Develop and implement individualized treatment plans in collaboration with the multidisciplinary team.
  • Evaluate and adjust treatment plans based on patient progress.
  • Offer support and guidance to junior staff and contribute to service improvement.

Key Requirements:
