We are looking for a dedicated and experienced Occupational Therapist to join our team in Tower Hamlets on a locum basis. This role offers a rewarding opportunity to make a difference in the lives of individuals transitioning from hospital care to community settings.

Key Responsibilities:

  • Conduct Care Act assessments for patients discharged from hospital to step-down beds, primarily in nursing home settings and occasionally in community rehab.
  • Collaborate with multidisciplinary team (MDT) professionals to develop tailored plans supporting patients’ return to their homes in the community.
  • Arrange appropriate care and support to facilitate safe and sustainable discharge plans.
  • Complete Continuing Healthcare (CHC) assessments, Mental Capacity Assessments, and functional assessments as required.
  • Maintain accurate documentation and adhere to legal and professional standards.

Requirements:

  • HCPC registration and professional qualification in Occupational Therapy.
  • Experience in hospital discharge, community rehab, or nursing home settings.
  • Strong knowledge of the Care Act and related assessment processes.
  • Ability to work effectively within a multidisciplinary team.
  • Excellent communication and organizational skills.
