Brief Job Description:

The post holder will spend 3 days in the admission avoidance therapy team working in the Emergency Department and Acute Medical Unit. You will be responsible for rapid assessment and provide a seamless discharge for patients working with community colleagues, local authorities and the voluntary sector.

For 2 days per week, you will work with the medical occupational therapy team to provide assessment, acute rehabilitation and support timely discharge working closely with the physiotherapy team and the ward MDT.
