- Build trusted relationships with care leavers to support their journey toward independence.
- Assess and meet their learning, personal, and social needs.
- Guide young people in areas such as healthy living, stable housing, and accessing educational or employment opportunities.
- Support care leavers with complex needs, including trauma, mental health, substance misuse, or offending behaviors.
- Manage safety and risks through assessments, safety planning, and effective interventions.
- Collaborate with internal and external services to ensure holistic support.
- Proven experience working with children, either as a Personal Advisor or within Children’s Social Care.
- Experience supporting care leavers or young people transitioning to adulthood.
- Knowledge of trauma-informed practice and understanding the challenges faced by care leavers.
- Strong interpersonal skills to build meaningful relationships with young people and stakeholders.
- Ability to assess risks and manage safety planning effectively.
- A proactive and creative approach to problem-solving and service delivery.
