Our Local Authority client are seeking an Occupational Therapy Care Manager (OTCM) to join the Community Independence Service (CIS) Reablement Team as a locum.

This is a dual role - Clinical OT and to support with managing clients care e.g completing a Care Act Assessment and Support Plan if assessed as needing ongoing care after Reablement period.

Key responsibilities:

  • Deliver skilled Occupational Therapy interventions to residents in their homes, supporting independence and recovery.
  • Manage a caseload for up to six weeks, creating and implementing goal-focused reablement plans.
  • Conduct holistic assessments to evaluate clinical and personal needs, ensuring comprehensive support.
  • Develop and execute intervention plans, including complex movement and handling assessments.
  • Recommend, order, and provide adaptive equipment to enhance independent living.
  • Collaborate with a multidisciplinary team to ensure coordinated and effective service delivery.
  • Maintain accurate and up-to-date case records using the Mosaic system.

Requirements:

  • HCPC-registered Occupational Therapist.
  • Experience in adult social care or community settings.
  • Experience in complex movement and handling assessments and care planning (training and supervision available).
