Are you an experienced Social Worker looking for a new role in South East London.

We are looking for Social Workerspost qualified experience in a front door, safeguarding or hospital team.

As an Adult Safeguarding Social Worker , you'll assess vulnerable adults' needs, promote independence, and ensure their safety and well-being, working with service users, carers, and multi-disciplinary teams to identify and respond to safeguarding concerns.

  • Key Responsibilities and Duties:

    • Assessments and Needs:

      Conduct timely and proportionate assessments of vulnerable adults' needs, focusing on outcomes they wish to achieve, and promoting independence and recovery.

    • Safeguarding:

      Identify and respond to safeguarding concerns, build managing safeguarding risks and benefits into care planning, and take action against those responsible for abuse or neglect.

    • Case Management:

      Manage and coordinate support plans for adults requiring frequent specialist intervention, promoting choice and control through personal budgets and direct payments.

    • Collaboration:

      Work as part of a multi-disciplinary team, collaborating with Social Inclusion and Recovery staff, health professionals, and other agencies to provide seamless services.

      This is a Hybrid role with 2 days ia week in the office.
