What You’ll Be Doing:

Join our Adult Social Care team as a Band 6 Discharge Social Worker, playing a key role in reducing hospital readmissions and ensuring smooth transitions from inpatient care to the community. Your key responsibilities will include:

? Conducting Care Act 2014 assessments to support patient well-being.? Working collaboratively with inpatient teams, community services, housing, and voluntary sectors.? Developing and implementing personalised discharge plans for high-risk individuals.? Providing follow-up support in the community to ensure sustained recovery.? Safeguarding and risk management to protect vulnerable adults.? Attending and contributing to discharge and bed management meetings.

Requirements & Qualifications:

To be successful in this role, you will need:

? A Social Work qualification with Social Work England (SWE) registration.? Experience in mental health social work, discharge planning, or community support.? Strong interpersonal and leadership skills to work across inpatient and community settings.? Ability to assess, plan, and coordinate care while working within multidisciplinary teams.
