What you will do:

  • Collaborate with a multi-disciplinary team to deliver holistic care assessments, promoting independence and wellbeing.
  •  Guide adults through reablement interventions, coordinating services and providing support within a client-led care plan.
  • Carry out comprehensive assessments including Mental Capacity Assessments following the Mental Capacity Act 2005.
  • Implement the Care Act 2014 in your practice, ensuring client welfare and safeguarding vulnerable adults.
  • Manage a diverse caseload, providing targeted advice and consultancy to peers and other stakeholders.
  • Maintain and update records as per Council Policy, ensuring accuracy and compliance with data protection legislation.
  • Actively participate in multi-disciplinary meetings, enhancing team collaboration and service delivery.
