Our Client Tower Hamlets Council, is looking for a Qualified Social Worker to join their Reablement Team.

The reablement team is a dynamic, occupational therapy led team with an emphasis on rehabilitation and enablement. It is the borough's first offer when residents require support.

The aim of the Reablement Team is to support a person to reach the maximum level of independence possible.

The Reablement Team also works closely with the Admission Avoidance Discharge Service (AADS), a health team with whom we are currently working on moving to more integrated ways of working.

Therapists work with local residents for up to 6 weeks to progress independence within activities of daily living.

To support this, reablement has its own in house reablement officers that provide day to day to support under the direction of an occupational/physio therapist, to encourage and support residents to carry out tasks themselves rather than carrying out tasks on their behalf.

As they near the end of their reablement intervention, some of our residents continue to have long term support needs.

With some residents, during or after the Reablement process, safeguarding concerns arise.

We are therefore looking for strength focused Social Workers to work as part of our multidisciplinary team to complete Care Act Assessments, support planning and safeguarding concerns.

This is presents an exciting opportunity to work within an integrated team with strong links to our health partners.

As an experienced social worker, you will manage a varied caseload, carrying out assessments focusing on the strengths of the person, their families, carers and local communities to promote independence.

You will be committed to working in a strength based, outcome focused way and to making a difference to the lives of the individuals and families which you work with.

Candidates must have post qualification experience along with a good knowledge of relevant legislation and local authority responsibilities in the safeguarding of vulnerable adults.
