About the Roles:
We are seeking three Practice Managers: two for our Adult Care Management Teams and one for our Adult Mental Health and Wellbeing Team (a s75 partnership between LBWF and NELFT). These positions are pivotal in ensuring high-quality, legally compliant social care practice while supporting and supervising qualified staff and social care assistants.
You will:
- Provide professional supervision and leadership to Social Workers and support staff.
- Undertake the full legal range of social care assessments and person-led support plans.
- Manage a small, complex caseload.
- Promote independence and well-being for adults and older people.
- Work collaboratively across internal departments, health services, and partner organisations.
Key Responsibilities:
As a Practice Manager, your role will involve:
- Supervision and Support: Overseeing team practice through regular supervisions, quality assurance, and case discussions to ensure high professional standards.
- Collaboration: Working with health services, partners, and statutory/non-statutory organisations to deliver holistic assessments, safeguarding, and care planning.
- Service Improvement: Identifying knowledge gaps, promoting best practices, and ensuring team adherence to quality standards.
- Case Oversight: Providing expertise on complex case management, including Safeguarding, Court of Protection cases, and DoLS/LPS.
- Leadership in Mental Health and Wellbeing Team: Supporting Social Workers alongside the nursing clinical lead, managing a small caseload, and contributing to clients' recovery journeys.
Qualifications and Requirements:
To apply, you must be a qualified Social Worker with current Social Work England registration and at least 2 years of post-qualified experience. Successful candidates will demonstrate:
- Comprehensive knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.
- Expertise in complex case management, including safeguarding, Court of Protection, and financial implications for commissioned care.
- Proven experience in supervising Social Workers and support staff.
- Strong IT skills and excellent verbal and written communication.
- Ability to promote alternative resources like S117 Rights, Continuing Health Care, and community resources.
