This is an exciting opportunity for a skilled Practitioner Manager to join the Adult Learning Disability Transforming Care Team in Lambeth for a 3-month locum position. This full-time role offers a competitive hourly rate of £40. As a key team member, you will work with adults with learning disabilities and/or autism who are at risk of specialist inpatient admission, aiming to prevent admissions and support swift transitions back into the community. What You Will Do:

  • Supervise up to 8 social workers and assessors in the Adult Learning Disabilities Team, providing guidance and support.
  • Ensure statutory assessments and reviews are completed within expected performance targets and quality standards.
  • Participate actively in the duty rota and be proactive in promoting anti-discriminatory practices.
  • Support council policy and work collaboratively with relevant agencies to aid in the transition from inpatient care to community living.
  • Provide your expertise in avoiding specialist inpatient admissions and expediting the discharge process for current inpatients.

Qualifications:

  • A relevant professional qualification in the field of Social Work..
  • Need to be a qualified social worker, registered with Social Work England and have significant post qualifying experience.
