- Conduct homelessness assessments and applications in accordance with homelessness legislation.
- Work closely with prisons, hospitals, housing services, and support agencies to facilitate effective resettlement.
- Provide housing advice, support, and advocacy to vulnerable individuals.
- Identify and assess the housing and support needs of clients and develop tailored resettlement plans.
- Collaborate with internal teams and external partners, including health and probation services.
- Maintain accurate records and case notes to ensure compliance with legislation and best practice.
- Experience in homelessness services, housing advice, or resettlement support.
- Strong knowledge of homelessness legislation and housing policies.
- Experience working with vulnerable individuals, particularly those leaving prison or hospital.
- Excellent assessment, case management, and advocacy skills.
- Ability to work effectively with multi-agency teams.
- Strong communication and interpersonal skills.
