Here are the need-to-know details:

• Job Title: Social Worker
• Band 6
• Location: Central London
• Duration: 3 months
• Schedule: 9-5 Monday to Friday 
• Start Date: ASAP
• Rate of pay: £27p/h umbrella days - Please note this will be paid too you via direct engagement with the trust in a PAYE equvilent rate. 
• Must holder a Valid Full UK Drivers License

Position Overview: We are seeking a skilled and compassionate social Worker to provide specialised care to individuals within the preparing for adulthood team and Transitions The ideal candidate will have a strong background in social work and a commitment to support planning and preparing for adult tasks.

Key Responsibilities:

Independent Clinical triage and facilitate social discharges from the acute.
Liaise directly with Local authorities, ICBs and internal stakeholders.
Experience in NHS and post qualification as a social worker, preferably in a IDH/D2A Model.
Monday to Friday- 9-5 pm
Experience in Acute discharges and support assessment in community triage referral from acute hospitals car driver -not essential. No home visits required. Office-based job with the base at Barley Lane
Mon-Fri- 9-5 pm Support junior staff as needed
Band 6 role 

Requirements and qualifications required for this role:

• Current and valid registration with the SWE.
• Proven experience as a social worker.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to providing high-quality patient care.
