We are looking for an agency nurse to carry out the following:

Key Tasks:1) Using available information assess 7 - 15 of the most expensive cases currently either solely or in the majority funded by the Local Authority to assess whether they meet the 2016 Guidelines.2) Report to nominated staff the conclusion of the assessment.3) Where applicable provide lines to take for Social Workers and Senior Management to re-refer such cases to the ICB where applicable.4) Agree any further follow up actions with nominated staffSupporting InformationThe top 20 high cost cases have been identified by the Local Council where they believe there is reasonable grounds to suspect that they children and young people (cyp) may be eligible for Continuing Care funding but which has been so far refused. We therefore anticipate completing a reassessment of health need on approx. 15 of these cases as part of a review of the funding relationship between the Local Council and the ICB. It is anticipated that each case may take up to 3 days to complete the assessment and report which will be a desk based process (i.e. no need to visit the cyp in person). In total the Council has committed funding for 45 days assessment (i.e. 15 cases at 3 days per case) and will review the outcomes following the completion of the task.We anticipate that no more than 3 Nurses may be required dependent on skills/experience (i.e. potentially 15 days each) and that this initial piece of work should be completed by 31 May 2025 at the latest. There is no expectation that a particular number of days will be worked each week and whilst hours are flexible consideration should be given to some hours being during 'office hours’ to enable follow up/information seeking to be completed with LBC staff (or others).Person SpecEssential1) Current registration with NMC (Nursing and Midwifery Council)2) Extensive experience of working with children/young people in either paediatrics or learning disability3) Understanding of the Children and Young People’s Continuing Care Guidelines (Dept of Health Feb 2016)4) Experience of completing Children and Young People’s Continuing Care assessments
