Community Care

  • Diploma / Degree in Occupational therapy.

  • UK NHS Experience

Seeking a dedicated and compassionate Occupational Therapist to join our Community Care team. This role involves providing high-quality therapy services to clients within their homes and community settings, fostering independence and enhancing their quality of life. The successful candidate will work collaboratively with clients, families, and other health professionals to develop and implement personalized care plans.

Key Responsibilities:

  • Client Assessment: Conduct comprehensive evaluations of clients' physical, psychological, and social needs within their living environments.
  • Care Planning: Develop and implement individualized therapy plans that focus on enabling clients to achieve their personal and functional goals.
  • Intervention Delivery: Provide hands-on therapeutic interventions to assist clients in developing, recovering, or maintaining daily living and work skills.
  • Adaptive Strategies: Recommend modifications to clients’ home or work environments and suggest adaptive equipment to improve daily functioning.
  • Education and Support: Educate clients and their caregivers on best practices to facilitate rehabilitation and safe living practices.
