Job OverviewWe are seeking an experienced Band 6 Occupational Therapist to provide specialist assessment and discharge planning support in an acute medicine setting. This role offers the opportunity to make a real impact on patients' lives while working within a dynamic and supportive team.
Key Responsibilities- Conduct comprehensive assessments to identify patient needs in acute medicine.
- Plan and implement effective discharge strategies, working with patients, families, and the multidisciplinary team.
- Provide expert advice on rehabilitation and ongoing care plans to ensure smooth transitions.
- Maintain accurate and up-to-date documentation in line with Trust standards.
- Participate in team meetings, case reviews, and service improvement initiatives.
- HCPC-registered Occupational Therapist.
- Experience in acute medicine, focusing on assessment and discharge planning.
- Strong clinical reasoning and decision-making skills.
- Excellent communication and teamwork abilities.
- Commitment to delivering patient-centered, high-quality care.
