Social Worker - Transformation Learning Disabilities Team

About the Role:

Randstad is seeking an experienced Social Worker to join the Transformation Learning Disabilities Team in Middlesbrough.
This role offers a unique opportunity to work with a team dedicated to transforming services for individuals with learning disabilities, promoting independence, and improving quality of life.
You will play a key role in ensuring that people with learning disabilities receive personalized care and support, enabling them to achieve their potential.

Key Responsibilities:

  • Carry out assessments to identify the needs of individuals with learning disabilities and create tailored support plans.
  • Work collaboratively with a multidisciplinary team, including health professionals, housing services, and families, to ensure a holistic approach to care.
  • Advocate for individuals with learning disabilities, ensuring their voice is heard and their rights are respected.
  • Support the development and implementation of innovative, person-centered services that enhance the lives of people with learning disabilities.
  • Monitor and review support plans regularly, adapting them as necessary to meet changing needs.
  • Stay informed on best practices and legislative changes, incorporating this knowledge into everyday practice to ensure high-quality care delivery.

Requirements:

  • Qualified Social Worker with valid Social Work England registration.
  • Proven experience working with individuals with learning disabilities.
  • Strong understanding of the Care Act, Mental Capacity Act, and other relevant legislation.
  • Ability to work both independently and as part of a team, with a proactive approach to problem-solving.
