Social Worker - Transformation Learning Disabilities Team

About the Role:

Randstad is seeking a dedicated and experienced Social Worker to join the Transformation Learning Disabilities Team in Middlesbrough.
This role offers an exciting opportunity to be part of a dynamic team focused on transforming services for individuals with learning disabilities, ensuring they receive the highest quality of care and support.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals with learning disabilities to identify their needs and develop tailored support plans.
  • Work collaboratively with a multidisciplinary team, including health professionals, educators, and families, to deliver person-centered care.
  • Advocate for individuals with learning disabilities, ensuring their rights and wishes are at the forefront of all decisions.
  • Facilitate and contribute to the development of innovative services and practices that enhance the lives of those with learning disabilities.
  • Monitor and review support plans regularly to ensure they remain effective and responsive to changing needs.
  • Keep accurate and up-to-date records in line with legal and organizational requirements.

Requirements:

  • Qualified Social Worker with a valid Social Work England registration.
  • Proven experience working with individuals with learning disabilities.
  • Strong understanding of current legislation and best practices related to learning disabilities.
  • A commitment to promoting equality, diversity, and inclusion in all aspects of work.
