Job Description:
As an Assessment Team Social Worker, you will play a key role in responding to referrals, conducting assessments, and working with families and children to ensure their safety and well-being.
You will work with a multi-disciplinary team to identify risks, create care plans, and make decisions that lead to positive outcomes for children and families.

Responsibilities:

  • Conduct timely and thorough assessments of children and families referred to the service.
  • Work closely with families to identify risks, strengths, and needs, ensuring appropriate interventions are in place.
  • Develop and implement child-centred care plans, ensuring safeguarding and promoting the welfare of children.
  • Collaborate with professionals from other agencies, including health, education, and housing, to provide holistic support.
  • Attend multi-agency meetings, case conferences, and court proceedings as needed.
  • Maintain accurate and up-to-date case records, adhering to legal and professional standards.

Qualifications:

  • Qualified Social Worker registered with Social Work England.
  • Experience in assessment, safeguarding, or statutory social work roles.
  • Strong knowledge of relevant legislation, safeguarding practices, and assessment frameworks.
