This will be working within an Adults Locality Team playing a crucial role in supporting individuals to live independently and with dignity. We are seeking a Qualified Social Worker to join this Adults Locality Team on a permanent basis. The successful candidate will be responsible for assessing, planning, and implementing care plans for adults with a range of needs. You will work collaboratively with other professionals to ensure the best outcomes for our service users.Your new role
- Conduct comprehensive assessments of service users' needs.
- Develop and implement personalised care plans.
- Provide support and advocacy for service users.
- Work in partnership with other agencies and professionals.
- Maintain accurate and up-to-date records.
- Ensure compliance with relevant legislation and policies.
What you'll need to succeed
- Degree in Social Work and registration with Social Work England.
- Proven experience in adult social care.
- Strong assessment and care planning skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Commitment to continuous professional development.
- Must be able to drive and have access to a car due to the rural location.
