About the RoleAs a Social Worker in Adults Social Care, you will assess and support adults with complex needs, disabilities, or vulnerabilities, ensuring they receive the appropriate care and interventions to enhance their quality of life.
Key Responsibilities- Manage a diverse caseload, working with adults who have physical disabilities, learning disabilities, mental health needs, or age-related conditions.
- Conduct assessments and care planning under the Care Act 2014, ensuring person-centred support.
- Work collaboratively with health professionals, care providers, and families to develop tailored care plans.
- Promote independent living and well-being, enabling individuals to maintain control over their lives.
- Provide safeguarding interventions and risk assessments to protect vulnerable adults.
- Ensure compliance with legislation, policies, and best practice guidelines.
? A qualified Social Worker, registered with Social Work England.? Experience in adult social care, safeguarding, or mental capacity assessments.? Knowledge of the Care Act 2014, Mental Capacity Act, and other relevant legislation.? Strong assessment, care planning, and risk management skills.? A commitment to empowering adults to live independently.
