This Complex Care Provider is looking for highly skilled and experienced Learning Disability and Paediatric nurse to join their expanding team of Nurses, Case Managers and Care assistants. They have a commitment to providing high-quality care to individuals with complex health needs, with an emphasis on patient-centered care, and collaborative approach to healthcare.
Job description:- Provide direct patient care to individuals with complex health needs.
- Collaborate with a multidisciplinary team to develop and implement care plans.
- Administer medications and treatments as prescribed.
- Monitor patient health and report any changes to the care team.
- Maintain accurate and up-to-date patient records.
- Educate patients and families on health management.
- Participate in quality improvement initiatives.
- Maintain a safe and therapeutic environment.
We're looking for you, if:
- You hold a nursing qualification in Learning Disabilities
- You have a current NMC PIN.
- You possess excellent communication and interpersonal skills.
- You are a compassionate and dedicated individual.
- You have a strong work ethic and commitment to teamwork.
- Experience in a similar role is highly desirable.
- £50-55K (Full time) per annum -depending on experience
