Job OverviewJoin the Community Rapid Response Team to provide urgent, patient-centered occupational therapy services. This role involves swift assessments and interventions aimed at preventing hospital admissions and supporting timely discharges.
Key Responsibilities- Conduct urgent assessments for patients in community settings.
- Develop and deliver individualised treatment plans to promote independence.
- Work closely with the multidisciplinary team to coordinate care.
- Support patients with complex needs in preventing hospital admissions.
- HCPC registration as an Occupational Therapist.
- Minimum of 6 months NHS experience in community or rapid response settings.
- Strong clinical decision-making and communication skills.
- Ability to work independently in a fast-paced environment.
