Connect2socialwork are urgently looking for a Social Worker to join their great Hospital Discharge with a Pay rate up to £35 an hour on a locum basis.

Job Description;

  • Work collaboratively to ensure safe and efficient hospital discharges.

  • Manage a complex caseload, working independently and efficiently.

  • Provide support and intervention for individuals and families during transition from hospital to community-based care.

  • Liaise with multidisciplinary teams to ensure seamless and person-centered care.

  • SWE Registration
  • Eligible to work in the UK
  • Drive and have access to a car
  • Post-qualified experience
  • Degree level or equivalent in Social Work
