Are you a dedicated RGN Qualified Nurse with a passion for complex care? Our client is seeking a Community Nurse Case Manager to support clients in the Midlands region. The ideal location for the Nurse Case Manager to live would be around Nottingham, to cover Lincolnshire, Staffordshire, and Midlands.
This role offers a unique blend of home-based work and community visits, ensuring clients can live independently at home and within their wider communities.

This role offers a competitive salary of £48,000 plus the opportunity to earn a bonus. You'll enjoy the flexibility of working from home two days a week, with the remaining three days spent visiting clients in the community. Additionally, you'll have the chance to make a real difference in the lives of those with complex care needs.

As a Community Nurse Case Manager, your responsibilities will include:
  • Conducting initial assessments in the community
  • Providing clinical oversight to your team
  • Collaborating with the wider multidisciplinary team (MDT)
  • Attending meetings and making necessary referrals
  • Managing a team of non-clinical case managers
