What you will do:
- Maintain a Complex Caseload: Assess, support plan, and review complex individual needs, initiating and coordinating outcomes to promote independence.
- Â Conduct Assessments: Use various health and social care tools, contributing to continuing healthcare assessments.
- Â Identify Reablement Opportunities: Provide access to reablement services as needed.
- Â Promote Strength-Based Approach: Embed the ethos of promoting independence and wellbeing, enabling individuals to meet their short- and long-term goals.
- Â Identify Community Resources: Maximize individuals' assets using preventative services and funding sources.
- Â Monitor and Review Services: Ensure coordinated service provision with a focus on wellbeing and outcomes.
- Â Liaise with Providers: Negotiate with local providers to deliver better outcomes.
- Â Provide Professional Support: Offer detailed information, advice, and support to individuals and their careers.
- Â Safeguarding Work: Undertake safeguarding duties as a Safeguarding Officer.
