What you will do:

• Maintain a Complex Caseload: Assess, support plan, and review complex individual needs, initiating and coordinating outcomes to promote independence.
•  Conduct Assessments: Use various health and social care tools, contributing to continuing healthcare assessments.
•  Identify Reablement Opportunities: Provide access to reablement services as needed.
•  Promote Strength-Based Approach: Embed the ethos of promoting independence and wellbeing, enabling individuals to meet their short- and long-term goals.
•  Identify Community Resources: Maximize individuals' assets using preventative services and funding sources.
•  Monitor and Review Services: Ensure coordinated service provision with a focus on wellbeing and outcomes.
•  Liaise with Providers: Negotiate with local providers to deliver better outcomes.
•  Provide Professional Support: Offer detailed information, advice, and support to individuals and their careers.
•  Safeguarding Work: Undertake safeguarding duties as a Safeguarding Officer.
