Placement available for one full time Social Worker in Community Prevention Team (Adult Social Care)

The Community Prevention Team works with citizens who have multiple long term conditions, which include citizens with neurological disabilities, and those who require ongoing "case management" in relation to their complex needs or support, in order to reduce the risk of avoidable admissions to hospital or long term care.

Colleagues in this team are linked with a primary care network (PCN) and work collaboratively with the multi-disciplinary teams (MDT) responsible for allocated citizens in that locality.

Pay rate £32 - £33 Umbrella per hour
