- Conduct safeguarding investigations, mental capacity assessments, best interest decisions, and outcome-focused assessments.
- Develop support plans to meet the care and support needs of individuals with Learning Disabilities/Autism and complex needs.
- Work within current legislation including the Care Act 2014, Mental Capacity Act 2005, Continuing Health Care, and the Mental Health Act.
- Collaborate with partner agencies to provide comprehensive services and support to individuals and carers.
- Provide support to individuals with physical disabilities, profound and complex needs, and those in the wider community.
- Work in a dynamic, diverse, and supportive team environment.
- Degree in Social Work, Dip SW, CQSW, or equivalent.
- Social Work England registration.
- Minimum 2 years post-qualifying experience in Adult Social Work, particularly with Learning Disabilities/Autism.
