About the Role
We are transforming our hospital discharge services to help Oxfordshire residents have their needs met in their own homes rather than in hospital. As part of our Home First D2A team, you will:

  • Support individuals to regain independence through a strength-based approach, working closely with reablement providers and the voluntary sector.
  • Assist with goal planning for short-term care after hospital discharge or to prevent hospital admission.
  • Use your expertise in activity analysis, assistive technology, and minor adaptations to reduce formal care.
  • Apply moving and handling skills as part of your role.

You are:
