Job Overview:

We are seeking a Band 7 Occupational Therapist to join the Transfer of Care Hub (TOCH) at St Richard's Hospital. This role involves optimising patient discharge pathways, reducing delays, and working collaboratively with acute and community therapy teams to implement the Home First approach.

Responsibilities:
  • Act as a therapy expert within TOCH, resolving therapy-related discharge delays.
  • Order necessary equipment to facilitate safe and timely discharges.
  • Collaborate with acute and community therapy teams to implement a pull discharge model.
  • Reassess patients where TOCH requests a review of Home First suitability.
  • Work with therapy leads to identify and address therapy-related delays.
  • Support transformation schemes to enhance discharge performance and patient flow.
Requirements:
  • HCPC-registered Occupational Therapist with NHS experience.
  • Previous experience in discharge planning and rehabilitation.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and ability to work within a multidisciplinary team.
  • Experience in managing therapy-related delays and implementing discharge improvements.
