Job Overview:We are seeking a Band 7 Occupational Therapist to join the Frailty Therapy Team at St Richard's Hospital. This role involves providing specialist assessment and intervention to individuals with physical, cognitive, social, or perceptual deficits that impact their functional ability and safety at home. The successful candidate will work within an integrated therapy model alongside Physiotherapists and the wider multidisciplinary team to ensure proactive rehabilitation and timely discharges with a strong focus on the Home First approach.
Responsibilities:- Provide OT support within the existing Frailty Therapy Team, working in an integrated therapy model with Physiotherapists.
- Work independently as an autonomous practitioner while collaborating with the wider multidisciplinary team.
- Conduct specialist assessments and interventions for patients with a range of functional impairments.
- Use an evidence-based, person-centred approach to improve patient independence and safety at home.
- Promote proactive rehabilitation to maintain and enhance function.
- Facilitate timely discharges, ensuring Home First principles are applied.
- HCPC-registered Occupational Therapist with NHS experience.
- Previous experience in frailty, rehabilitation, or acute settings.
- Strong clinical assessment and decision-making skills.
- Excellent communication and teamwork skills within an integrated therapy model.
- Ability to work independently and manage a complex caseload effectively.
