Physiotherapist- Band 6

Location: Rhyll

Start date: ASAP End date: Ongoing

  • Mon-Friday 37.5 hours a week
  • B6 level. working in an acute frailty unit
  • The Acute Frailty Physiotherapist is responsible for providing specialized physiotherapy assessment and intervention to older adults and individuals with frailty syndromes in an acute hospital setting.
  • The role aims to promote independence, optimize functional mobility, and support safe discharge planning for patients who may have complex health and social care needs.
  • The physiotherapist will work closely with a multidisciplinary team (MDT), including geriatricians, nurses, occupational therapists, and social workers, to deliver person-centered care and improve patient outcomes.
    • Conduct comprehensive physiotherapy assessments, including functional mobility, strength, balance, and cognitive assessments, tailored to the frail and older adult population.
    • Develop and implement individualized treatment plans, setting realistic and achievable goals in collaboration with patients, families, and caregivers.
    • Monitor and adjust treatment interventions based on patient progress, ensuring evidence-based and safe practices are adhered to in all clinical interventions.
