- Engaging with referral agencies, third-sector organizations, and community groups to generate referrals for services.
- Conducting initial assessments to evaluate service users' needs, risks, and suitability for the program.
- Designing and delivering structured support plans, referring to specialist agencies as required.
- Maintaining regular contact with service users, providing professional guidance and acting as a positive role model.
- Conducting risk assessments and ensuring appropriate actions are taken for safety.
- Encouraging service user participation in decision-making and service development.
- Building relationships with external agencies to provide additional support for service users’ needs.
- Experience conducting needs and risk assessments for service users.
- Strong knowledge of housing support services and welfare benefits.
- The ability to develop and implement support plans tailored to individual needs.
- Experience working in partnership with external agencies and community groups.
- Excellent communication and record-keeping skills.
- 35 hours per week, Monday to Friday, 9 AM – 5 PM
- 3-month contract, with potential for a temp-to-perm opportunity
- £16.95 per hour (LTD) / £14.45 per hour (PAYE, inclusive of holiday pay)
