We are looking for an experienced Team Leader to join our care home in Ruislip, leading a team to provide high-quality, person-centered care.

Key Responsibilities:
  • Oversee medication management, including ordering, administration, auditing, and record-keeping.
  • Develop and update care plans using Person-Centered Software (PCS).
  • Make referrals to GPs, district nurses, dietitians, and other healthcare professionals.
  • Lead and support a team of up to six HCAs and ancillary staff.
  • Ensure compliance with risk assessments and care file management.

  • Experience in a leadership role within a care setting.
  • Strong medication management and care planning skills.
  • Excellent communication and written English skills.
