Main Purpose of the Role


  • To contribute to the creation, implementation and review of individual care/ support plans for children, young people and their families to meet their assessed need.
  • To work with partners and colleagues in determining what additional needs families may have and to act as Lead Professional where appropriate
  • To act within the principles of Halton's model of early help, identifying needs within families including those aged 0-18yrs.

To plan, implement and review action plans to meet assessed needs, in conjunction with families and partners, preventing needs from escalating wherever possible.


Key Duties

Act as key worker to a number of children and families, working in partnership with them and other relevant professionals to identify needs and co-ordinate/implement the individual care/ support plans.

Devise, monitor and review individual care/ support plans to the standards identified in the service specification.

Ensure that the personal care needs of the children and young people are met.

Liaise and communicate with other agencies, parents and professional staff so that appropriate resources may be identified and mobilised for the benefit of children, young people and their families.

Maintain effective and accurate record systems to enable the preparation of necessary reports to contribute to the care planning process.

Participate in the corporate administration of the service both within the Team/Centre and in the community.

Where appropriate, offer advice and support to other staff, to ensure the effective and efficient organisation of the service.

Be aware of and comply with the statutory requirements, group policies, equal opportunities and other regulations and procedures- especially in respect of safeguarding and Early Years developments- bringing any safeguarding concerns to the attention of the Senior Early Help Officer/ Principal Manager.

Education / Qualifications

Essential


NVQ Level 3 in Social Care or equivalent

A minimum of three GCSE's or equivalent at grade C or above which include maths and English or demonstrate the appropriate level of skills or experience

Skills & Abilities


Experience of working with children and families in a social care setting, or related environment

Working knowledge of child and adolescent development.

Effective communication skills with children, young people with additional needs and their families.


Experience of using information technology.

Working knowledge of relevant government legislation, guidance and local procedures relating to children and families, including early help and safeguarding.

Good written and verbal skills.


Experience of working collaboratively with colleagues from other organisations, including social care, health, education, voluntary sector

Working knowledge of services for disabled children aged 0-5 and their families

Good assessment skills
