We are offering an exciting opportunity for a Social Worker to join the North Locality Team within the Adult Social Care & Homelessness Service in beautiful Ruthin. This ongoing role pays £33 hourly and requires a full-time commitment of 37 hours per week. What you will do:

• Conduct appropriate and proportionate assessments with individuals and their families to help achieve realistic and desirable outcomes.
• Collaborate effectively within a multidisciplinary team comprising Social Workers, Occupational Therapists, and other social care staff.
• Actively engage in partnership working to ensure seamless service delivery within the community.
• Manage and prioritise your own workload efficiently while demonstrating self-motivation and initiative.
• Maintain professional registration with the Care Council for Wales and commit to continuing professional development.
