Are you a dedicated Physiotherapist with a passion for making a difference in your community? MFT are currently seeking a passionate and skilled Senior Community Physiotherapist to join the team at the Trafford Community Rehabilitation team.

This role involves conducting initial assessments, delivering specialist treatment plans for patients with complex medical conditions, and working collaboratively with other healthcare professionals to ensure optimal patient care. You will play a key role in evaluating and modifying treatment plans, supporting service improvements, and mentoring junior staff.

Main duties of the job

  • Perform initial and specialist assessments to aid in medical diagnoses and formulate treatment plans.
  • Plan, deliver, evaluate, and modify treatment programs for patients with diverse needs.
  • Provide guidance to patients, carers, and junior team members.
  • Manage a varied caseload, applying advanced clinical reasoning.
  • Lead case conferences and participate in the service's quality improvement initiatives.
  • Mentor Band 5 Physiotherapists, Patient Support Workers, and students.

To be successful in applying for this role you will need:

  • Degree in Physiotherapy and current registration with the relevant professional body.
  • Experience in managing complex cases in a community setting.
  • Excellent communication skills for patient and team interactions.
  • Ability to work independently and manage a busy caseload.
