Are you passionate about making a difference in the lives of young people with Special Educational Needs and Disabilities (SEND)? We’re looking for a full-time SEND Lead Worker to join our team on an initial 3-month contract (with the potential to extend).

About the Role:

As a Preparation for Adulthood SEND Lead Worker, you will:

  • Manage and support a caseload of young people, ensuring their wellbeing and development.
  • Coordinate care packages and support plans to meet individual needs.
  • Act as the main point of contact for young people and their families, providing expert guidance.
  • Lead the Education, Health, and Care (EHC) Needs Assessment process.
  • Work closely with professionals to promote inclusion in mainstream settings.
  • Support families and professionals with SEND-related advice and resources.
  • Ensure a holistic, person-centred approach to achieving the best outcomes for children and young people.

What We’re Looking For:

We need dedicated professionals with experience in SEND support, strong communication skills, and the ability to coordinate multi-agency services to improve outcomes for young people.
