Job Description: Home Support Service Coordinator

Fixed Term Contract until April 2025

Hours of work: 28-30 hours per week - 26,805

Location: Solihull

I currently have a very exciting opportunity to join one of the leading elderly charities within the UK. They are looking for a Home Support Service Coordinator to work out of their Solihull Site.

The Home Support Service Coordinator is responsible for overseeing and coordinating the delivery of home support services to clients within Solihull. This role involves ensuring that clients receive high-quality, individualized care that meets their specific needs and preferences.

**Responsibilities**

* **Client Assessments:** Conduct thorough assessments of new clients to identify their unique needs, preferences, and support requirements.

* **Care Plan Development:** Develop personalized care plans based on client assessments, outlining specific services and goals.

* **Caregiver Matching:** Match clients with suitable caregivers based on skill sets, availability, and compatibility.

* **Caregiver Training:** Provide ongoing training and support to caregivers to ensure they are equipped to deliver high-quality care.

* **Quality Assurance:** Monitor the quality of care provided by caregivers through regular checks, feedback, and audits.

* **Scheduling:** Create and manage caregiver schedules to ensure optimal coverage and efficient service delivery.

* **Communication:** Maintain open and effective communication with clients, caregivers, families, and other stakeholders.

* **Crisis Management:** Respond to and resolve any crises or emergencies that may arise.

* **Documentation:** Maintain accurate and up-to-date documentation of client records, care plans, and incident reports.

* **Compliance:** Ensure compliance with all relevant regulations, policies, and procedures.

To be considered for this role then you must be a DRIVER, local to the Solihull area and have experience in the social care industry. It would be an advantage if you have been a care coordinator before.
