  • Undertake person-centred assessments to empower individuals, promoting their health, wellbeing, and independence.
  • Work collaboratively with individuals who have complex needs to develop support plans that maximise their abilities.
  • Contribute to the performance, quality assurance, and improvement of Adult and Integrated Care Services.
  • Ensure services are responsive to both local and national landscapes, safeguarding adults while promoting health and independence.
  • Facilitate choice and independence through person-centred care assessments, reallocating resources as needed.
  • Connect individuals and carers with universal and non-care services, providing guidance and advice.
  • Review support packages consistently to ensure the right level of support is being delivered.
  • Manage a diverse caseload, offering solutions for adults with complex challenges.
  • Represent the service in court proceedings if required and conduct Safeguarding Adults Investigations in line with procedures.
  • Engage in training and development activities to maintain professional standards and registration with HCPC.
  • Responsible to: Operations Manager for overall performance and accountable to the Assistant Manager for casework practice.
