£43,680 + bonus
As a Nurse Case Manager ( Nurse / RGN / RCN ) , you will manage a caseload of Nurses and Care Support Workers, ensuring the delivery of high-quality care. You'll work closely with the office team to plan workloads and duties efficiently, supporting the day-to-day operations and handling administrative tasks.
Your role will involve supporting the development of Registered Nurses and Care Workers, providing clinical training, supervision, and competency sign-offs. You will also act as a key link between commissioners, clients, and the organization, liaising with other professionals when required.
In this role, you will provide clinical leadership to clients and staff by prioritising individual needs and conducting regular reviews. You'll be involved in the full care planning process-assessment, planning, implementation, and evaluation. You will also offer clinical input through client visits and staff support across different settings.
Attending review meetings, adult/child protection meetings, and coordinating with Care Managers, Social Services Managers, and Primary Care professionals will be part of your responsibilities.
Experience required:
- Experience caring for: Epilepsy, gastrostomy, tracheostomy, spinal injury, respiratory conditions, learning disabilities
- NMC PIN - Registered Children's Nurse or RGN with paediatric experience
