Job OverviewWe are seeking a dedicated Band 6 Occupational Therapist for a community-based role at MPFT. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.
Key Responsibilities- Assess, plan, and deliver occupational therapy interventions for patients in the community
- Work within multi-disciplinary teams to support rehabilitation and recovery
- Develop and implement tailored care plans to meet individual needs
- Provide advice and education to patients, families, and carers
- Manage a caseload, ensuring timely and effective interventions
- HCPC registration as an Occupational Therapist
- Proven experience in community-based occupational therapy
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Experience in managing complex caseloads is desirable
