Key Responsibilities:

  • Provide high-standard physiotherapy interventions for pulmonary rehabilitation in a variety of community settings (e.g., community centres, health centre's).
  • Therapeutic assessment for patients with long-term respiratory conditions, delivering individualized treatment plans as an autonomous practitioner.
  • Work independently, unsupervised, during community visits, managing a caseload and meeting the needs of the Community Respiratory Service.
  • Participate in pulmonary rehabilitation audit and research alongside senior therapists.
  • Deliver patient education talks for pulmonary rehabilitation programs under the guidance of senior therapists.
  • Use assessment tools and outcome measures to monitor progress and outcomes.
  • Collaborate effectively with the multi-disciplinary team (MDT), patients, carers, line managers, and external agencies.
  • Undertake evidence-based audits to evaluate team clinical practices and make recommendations for improvements.
  • Supervise and peer review clinical work, with regular support and guidance from senior physiotherapists.
  • Experience:
    • NHS experience is essential, including a range of placements at graduate/undergraduate level.
    • Prior experience in Pulmonary Rehabilitation settings is highly desirable.
  • Skills & Abilities:
    • Ability to work effectively as part of the multi-disciplinary team (MDT).
    • Excellent communication skills with patients, carers, colleagues, and external agencies.
    • Ability to work autonomously and independently during community visits.
    • Knowledge and experience with assessment tools and outcome measures in respiratory care.
    • Ability to travel frequently throughout the borough within the working day.
    • Experience in providing evidence-based clinical audits and making recommendations for service improvements.
    • Strong ability to deliver patient education in a clear, empathetic manner.

Additional Requirements:

  • Car driver is essential for community visits.
  • Ability to work flexibly, including a 5-day work week over a 7-day period as required by service demands.
