  • Empower Individuals - Create tailored support plans that help people regain control of their lives and achieve long-term stability.
  • Respond to Urgent Needs - While remote work is part of the role, on-site visits will be required when urgent care is needed.


What You Need:

  • Specialist Experience - A strong background in hospital discharge or intermediate care, allowing you to make informed decisions that improve lives.
  • Expertise in Mental Capacity & Safeguarding - Confidence in conducting assessments and handling safeguarding concerns effectively.
  • Social Work England Registration & ASYE Completion - A qualified professional with proven experience in statutory social work.
  • Legislative Knowledge & Best Practices - A strong understanding of key frameworks, policies, and approaches that drive effective adult social care.
