What You'll Be Doing:
- Assess & Safeguard - Conduct Mental Capacity Assessments to ensure individuals receive the right support while maintaining their dignity and independence.
- Work in a Multi-Disciplinary Team - Collaborate with health and social care professionals to provide joined-up, person-centred support that leads to better outcomes.
- Empower Individuals - Create tailored support plans that help people regain control of their lives and achieve long-term stability.
- Respond to Urgent Needs - While remote work is part of the role, on-site visits will be required when urgent care is needed.
What You Need:
- Specialist Experience - A strong background in hospital discharge or intermediate care, allowing you to make informed decisions that improve lives.
- Expertise in Mental Capacity & Safeguarding - Confidence in conducting assessments and handling safeguarding concerns effectively.
- Social Work England Registration & ASYE Completion - A qualified professional with proven experience in statutory social work.
- Legislative Knowledge & Best Practices - A strong understanding of key frameworks, policies, and approaches that drive effective adult social care.
- Driving Licence & Access to a Car - While exceptions apply under the Equality Act, being mobile allows for greater flexibility in responding to cases.
