Social Worker, £35ph, Leaving Care, SwindonPertemps are recruiting for qualified and experienced Social Workers to join the Leaving Care Service. The Leaving Care Service links in early with the young people transferring for other services and the postholder will lead on the completion of pathway plan assessments and the initial pathway plan. It will remain the postholder’s ongoing responsibility to maintain the plans in line with the required legislation up until the young person is 18 years old when they will then fully transfer to their allocated Pathway Advisor.
