Job OverviewAtlantis Medical is seeking a dedicated Band 6 Occupational Therapist to join the Dementia Services team at Midlands Partnership NHS Trust. This role involves providing specialist assessment and intervention for individuals living with dementia within their own homes. You will work autonomously while receiving support from the Clinical Lead, OT colleagues, and a multidisciplinary team.
Key Responsibilities- Conduct home visits to assess patients' needs and provide tailored interventions.
- Develop and implement individualized care plans to promote independence and well-being.
- Work closely with families, carers, and other healthcare professionals to provide holistic care.
- Support patients with cognitive and functional assessments, identifying strategies to enhance daily living.
- Contribute to risk assessments and safeguarding procedures for vulnerable adults.
- Collaborate with the wider Community Mental Health and Dementia team to ensure effective service delivery.
- Maintain accurate and up-to-date patient records in line with NHS guidelines.
- HCPC registered Occupational Therapist with relevant qualifications.
- Experience working in community mental health and dementia services.
- Ability to work autonomously with minimal supervision.
- Strong assessment and problem-solving skills.
- Excellent communication and team-working abilities.
- Full UK driving license and access to a vehicle (preferred for community visits).
