We are looking for a Pharmacist team leader in the women's and Children's department within the NHS at a hospital in West London.

Job summary

The post holder is an experienced clinical pharmacist with specialist knowledge, highly developed management and leadership skills, who will manage the Pharmacy service provision and lead the medicines optimisation agenda to the Women & Children service.

  1. critically evaluating and interpreting patient information (prescription charts, patient notes, observation charts, laboratory results and medicines) to ensure that prescribed treatment is safe, effective and appropriate for the patient.
  2. proactively suggesting evidence-based improvements to drug therapy and management of patients to consultants and senior medical staff, anticipating a positive outcome for the patient.
  3. providing evaluated information to consultants, senior medical staff, nursing staff, pharmacists and other health care professionals, making recommendations where opinions differ, and information is lacking.
  4. counselling patients, relatives and carers about medication, checking their understanding of information and being mindful of physical and language barriers
  5. promoting cost-effective prescribing by encouraging and supporting compliance with the Drugs Guide and by advising medical, nursing and other healthcare professionals including pharmacy staff, in line with Trust, local and national policies and procedures
  6. feed back to the Chief Pharmacist and other members of the team, changes that impact on current practice.
