On behalf of Walsall Council, Opus People Solutions have an exciting opportunity for a Social Care Facilitator to join their team.

Job Title: Social Care Facilitator
Length of assignment: 3 Months with the possibility to be extended
Hours per week: 37 hours
Hourly rate: 18.26ph PAYE / 24.76ph umbrella
Location: Walsall Council - Blakenhall Village Centre, Walsall WS31LZ
Remote/Office based: Hybrid working, minimum 2 days office based


In this role you will be

Main Purpose:

You will support vulnerable adults through self-directed support and personalisation, meeting care management and support needs as part of a multi-skilled team.

Key Responsibilities:

  • Assist in creating support plans, promoting choice, independence, and community access.
  • Contribute to assessments and specialist evaluations in line with self-directed support principles.
  • Promote citizens' health, wellbeing, and decision-making, ensuring mental capacity considerations.
  • Collaborate with service users, carers, and multi-disciplinary teams.
  • Identify needs for simple assistive equipment and refer for more complex needs.
  • Maintain accurate records and adhere to relevant policies, including the Mental Capacity Act and Health and Safety.
