This job opportunity is with a leading Rehab and Injury case management specialist company in the UK.
The position is available across England and offers a salary of up to £75 per hour, depending on experience.
Candidates must have a minimum of 2 years of case management experience and hold a valid HCPC or NMC PIN.
Key responsibilities include conducting initial assessments, supporting clients in achieving their goals, and facilitating their return to work.
Applicants must have UK experience, a valid UK driver's license, and the right to work in the UK, as they cannot offer working visas.

This opportunity is working for a Rehab and Injury case management specialist company They are one of the UKs largest independent providers of rehabilitation case management services.

Job locations Available: ENGLAND WIDE

Up to £75 an hour - experience dependant

Monday - Friday 9 am - 5 pm

MUST BE A CASE MANAGER WITH 2 YEARS EXPRINECE - with a HCPC OR NMC PIN

Duties:

- Carrying out initial assessments

- Support Clients to identify & achieve full potential or focussed goal

- The Aim would be to facilitate a return to work or the best possible functionality

- Set out a road map for recovery

- Identify any potential barriers to recovery

- Point of Contact to client/patient

- working in conjunction with family members, employers, Medical professionals & Insurance organisations

- Implementing & Monitoring.

Must haves to apply:

UK experience and registration with HCPC

Right to work in the UK They cannot offer working visas

Must have own car with valid UK driver's license

- 2 years Case Management exp
