Rehabilitation Care worker

Overview:We are seeking compassionate and dedicated Care Workers to join our rehabilitation team. As a Rehabilitation Care Worker, you will provide essential support and assistance to individuals in their own homes on discharge from hospital with the aim of promoting independence and re-enablement. Responsibilities:

- Following personalised care plans to maximise independce- Assist individuals with daily activities such as personal hygiene, dressing, and meal preparation- Administer medication as directed by healthcare professionals- Provide emotional support and companionship to individuals in your care- Assist with light housekeeping tasks, including cleaning and organizing living spaces- Help individuals with mobility issues, using proper lifting and transferring techniques- Communicate effectively with individuals, their families, and healthcare professionals- Utilize IT systems to accurately document care provided

Qualifications:- Fluent in English, both written and verbal- Previous experience in a care home or home care setting is preferred- Basic IT skills for documentation purposes- Ability to follow care plans effectively- Excellent communication skills to interact with individuals, families, and healthcare professionals- Valid driver's license and access to own vehicle.
